A choice of SeniorBlue HMO Plan options
SeniorBlue HMO offers many plan choices that provide a variety of co-payment options for various types of medical services. With many plans to choose from, there is sure to be one that meets your needs and budget. Our SeniorBlue HMO plan options provide all of the coverage, physician, facility, and prescription drug, in one place while eliminating the need to purchase a separate Medicare Part D Prescription Drug Plan. NOTE: You must continue to pay your Medicare Part B premium.
No Annual Plan Deductible
There is no annual deductible to satisfy before your SeniorBlue HMO benefits begin, no matter which plan you choose. Your coverage will begin the first day of the month after your completed enrollment application has been received.
Low out-of-pocket expenses
No matter which option you select, office visits to your family doctor are covered at a low co-payment. As long as you coordinate your care through your Primary Care Physician (PCP), you simply pay a co-payment for doctor visits, medically necessary hospital stays and diagnostic tests.
Primary Care Physician (PCP) Selection
You have the freedom to choose a Primary Care Physician (PCP) from our broad network of physicians.With one of the largest hospital, physician and medical professional networks in our area, chances are your family doctor already participates. Access a directory of network providers.
You must use plan providers except in emergent or urgent care situations. If you obtain routine care from out-of-network providers neither Medicare nor SeniorBlue HMO will be responsible for the costs.
Health Education and Preventive Benefits
SeniorBlue HMO understands that preventive care services such as routine physicals and flu shots are important to maintaining your health. We provide coverage for these types of services – beyond Original Medicare’s deductibles and copayments. You do not need your PCP to coordinate your visits for routine dental, routine vision and fitness benefits. You may even self-refer to a network provider for annual routine services such as mammograms, and pelvic/pap exams. Download the Adult Health Maintenance Guidelines.
Dental and Vision Benefits
Depending on the plan option you choose, dental and vision benefits may be available. Refer to the 2014 Summary of Benefits for more information.

When you travel, SeniorBlue HMO provides 24-hour coverage for emergencies across the nation and around the world.
24-Hour Nurse Line
Questions about your health can come up at any time, day or night. That’s why you’ll appreciate easy access to our Nurse Line, a trusted source for health information 24-hours a day, 7 days a week. With Nurse Line, simply pick up the phone and you can speak to a registered nurse. And, the best part...this service is provided at no additional cost to members. 2014 STAR Ratings
Medicare evaluates plans based on a 5-Star rating system. Star Ratings are calculated each year and may change from one year to the next.
